Here is the plot description for “Captain America: The First Avenger”:
“Captain America: The First Avenger” focuses on the early days of the Marvel Universe when Steve Rogers (Chris Evans) volunteers to participate in an experimental program that turns him into the super soldier known as Captain America.
As Captain America, Rogers joins forces with Bucky Barnes (Sebastian Stan) and Peggy Carter (Hayley Atwell) to wage war on the evil HYDRA organization, which is led by the villainous Red Skull (Hugo Weaving).
One of a continuing slate of films being produced by Marvel Studios based on the Marvel characters, “Captain America: The First Avenger” joins “Thor” (which was released on May 6, 2011) and “The Avengers” (which is scheduled to be released on May 4, 2012).

Steve Rogers was the first (and only) given the Super-Soldier serum, as the man who created it was killed by a Nazi. The formula and procedure died with Abraham Erskine. Steve Rogers was the only super-soldier and he would be America’s superhero. He killed the Nazi spy, then vowed to use his power to fight the enemies of America and become a symbol of freedom. The U.S. gov’t, after the loss of their planned super-soldier army, decided to use Rogers to the fullest potential. He would become the symbol of freedom and hope and the main weapon against the Nazis Red Skull. They gave him a costume based upon the American flag, an unbreakable shield, a sidearm, and named him Captain America.

Answer: Steven Rogers got the name Captain America by the United States government after he became a super human super soldier as a result of being a part of an experiment that injected him with the Super Soldier formula.
This is the story the comic would have you to believe. The real answer is that the creators of the comic made up the most patriotic name imaginable for a new hero because at the time patriotic super heroes had become all the rage due to the war.
