This submission form is for the HollywoodChicago.com Hookup for 25 admit-two run-of-engagement “Crazy Heart” movie passes. The passes are good at any Chicago-area Landmark Theatre at the time of your choosing.

To be eligible, you must submit a valid entry in this confidential, Web-based form. “Crazy Heart,” which opened in Chicago on Dec. 25, 2009, stars Jeff Bridges, Maggie Gyllenhaal, Robert Duvall, Brian Gleason, Rick Dial, William Sterchi, Richard W. Gallegos, William Marquez, LeAnne Lynch, David Manzanares, J. Michael Oliva and Ryil Adamson from writer and director Scott Cooper. “Crazy Heart” is based on the novel by the same name from Thomas Cobb.

The winners will be selected in a random drawing. You will be contacted if you win. The “Crazy Heart” movie passes will be mailed to the winners at our expense.
