This submission form is for the HollywoodChicago.com Hookup for 10 VIP IMAX “Transformers” tickets with one lucky winner being selected for an advance screening for two of “Transformers: Revenge of the Fallen”. To win, you must be among the 10 people selected as true Transformers fans from your submission in this confidential, Web-based form.
