This submission form is for the HollywoodChicago.com Hookup for 3 “Alpha and Omega” VIP prize packs. Each prize pack includes a Nintendo DS game for “Alpha and Omega,” a run-of-engagement movie pass (see the film where you want, when you want), lenticular bookmark, activity sheets and a stuffed wolf!

“Alpha and Omega” features the voices of Hayden Panettiere, Dennis Hopper, Danny Glover, Justin Long, Christina Ricci, Larry Miller, Eric Price, Vicki Lewis, Chris Carmack, Kevin Sussman, Brian Donovan and Bitsie Tulloch from directors Anthony Bell and Ben Gluck and writers Chris Denk and Steve Moore. The film opened in theaters on Sept. 17, 2010.
