This submission form is for the HollywoodChicago.com Hookup for 10 “The Expendables” admit-two run-of-engagement movie passes. The passes are good at any participating Chicago theater at the time and date of your choosing during the film’s run.

To be eligible, you must submit a valid entry in this confidential, Web-based form. You must have a HollywoodChicago.com site account and include your first and last name, e-mail address and physical mailing address when submitting this form. Your personal information, which will not be sold or shared elsewhere, will be kept strictly confidential and is only for awarding winning entries. “The Expendables” stars Sylvester Stallone, Jason Statham, Jet Li, Mickey Rourke, Dolph Lundgren, Steve Austin, Eric Roberts, Randy Couture, Terry Crews and David Zayas. The film opened on Aug. 13, 2010. The winners will be selected in a random drawing. You will be contacted if you win. Our “The Expendables” movie passes will be mailed to the winners at our expense.
