This submission form is for the HollywoodChicago.com Hookup for 10 “Middle Men” soundtracks and 40 admit-two passes to the early Chicago screening of the film.

“Middle Men” stars Luke Wilson, Giovanni Ribisi, Kelsey Grammer, James Caan, Laura Ramsey, Terry Crews, Stacey Alysson, Diane Sorrentino, Gabriel Macht, Christopher McDonald and Rade Serbedzija from writer and director George Gallo. The film opens on Aug. 6, 2010.

To be eligible, you must submit a valid entry in this confidential, Web-based form. You must have a HollywoodChicago.com site account and include your first and last name, e-mail address and physical mailing address when submitting this form. Your personal information, which will not be sold or shared elsewhere, will be kept strictly confidential and is only for awarding winning entries.
